Patient Forms

If you are a new patient to our office OR a patient coming in for a 6 month cleaning appointment, click the Patient Forms link below which contain the forms that will need to be filled out when you arrive at our office.  By completing them prior to your appointment, they will be automatically downloaded into our system before you arrive which will allow us to attend to your child's medical needs more quickly than completing them upon arrival.  Thank you and please call our office if you have any questions at all.

CLICK HERE» Patient Forms- These forms are used for BOTH Current patients to complete an updated health history and New Patients.  If you are a new patient to the office or have never completed online forms here on the web page or on the tablet in the office, please be sure to complete both the Consents and the Medical History Form.  If you have already completed an update or new patient forms on the website or in the office on the tablet, you only need to complete the section "Medical History Form" located within this patient forms section, as we already have the consents on file electronically.
